Community Neuro Rehabilitation Team

The Community Neuro Rehab Team (CNRT) provides a multidisciplinary goal orientated approach and holistic individualised care for people who have complex neurological conditions.

The team assess and treat people over 16 who have been formally diagnosed with a stroke and are registered with a GP in Plymouth, South Hams & West Devon.

The team also assess and treat patients with complex neurological diagnoses who are registered with a Plymouth GP and are due to expand this service to include South Hams & West Devon in the future.

Highly specialist multidisciplinary rehabilitation is provided for people with a neurological impairment living in the community; focusing on recovery, prevention of deterioration and/or self-management and seek to promote independence and improve quality of life.

The service works closely with the University Hospitals Plymouth bed base and the Plym Neuro Rehab Unit within Livewell Southwest, to facilitate earliest possible hospital discharge, facilitating an in-reach system, as appropriate to ensure smooth transfer of care for inpatients.

The service consists of Rehabilitation Consultants, Specialty Doctors, Physiotherapists, Occupational Therapists, Speech and Language Therapists, Neurological Rehabilitation Assistants, Nurses, Referral Coordinator and administration support.

Regular contact is made with other services including the Plym Neuro Rehabilitation Unit, Acute and Community Stroke Rehabilitation Units, Neuropsychology, Community Therapy Teams, Social Workers and Social Care services and can access support/input from Orthotics, wheelchair services, and Dieticians. The team also have links with specialist nurses eg MS and Parkinsons and District Nurses as required. In-reach to some local hospital settings is supported to facilitate smooth transfer of care.

The service has an office base and access to treatment areas at the Local Care Centre, Mount Gould Hospital. Patients are treated within their own place of residence, in the community and/or in the Therapy Gym based at Mount Gould Hospital.

Café and Toilet Facilities

There are toilets and a café located in the Local Care Centre reception area.

Café opening hours:  09:00-15:00.

The Mount Gould Hospital Site is a ‘no smoking’ zone.

Driving, Parking and finding the Local Care Centre

There are free parking spaces designated for visitors on the Mount Gould Site.

if coming from the A38, Mount Gould can be accessed from the Embankment Road turn-off.

Finding the LCC

There is signposted access directly from the covered car park underneath the Local Care Centre. This has stairs or a lift into the main Local Care Centre reception.

If walking, there is pedestrian level access to the Local Care Centre reception, from the Mount Gould Road.
